MACULAR AND CHOROIDAL TOMOGRAPHIC THICKNESS EVALUATION IN PATIENTS THAT UNDERWENT PHACOEMULSIFICATION WITH THE USE OF INTRAOPERATIVE EPINEPHRINE VERSUS WITHOUT ITS USE

Introdução / Purpose

Phacoemulsification (PHACO) is the most performed ophthalmologic surgery in the world. Pharmacological mydriasis starts in the immediate preoperative and sometimes the use of epinephrine intraoperative in the anterior chamber is needed once miosis may occur. Inappropriate moves and prolongated surgical time may provoke early miosis. Inflammatory mediators have a role in some posterior segment pathologies and some studies suggest that intraoperative epinephrine may also cause alterations in retina or choroidThe aim of this study was to evaluate the influence of the use of epinephrine intraoperative on choroidal and macular thickness that could lead to macular edema in patients that underwent phaco surgery.

Material e Método / Methods

Seventy patients with surgical indication of phaco and without past ocular history of any retinal pathologies such as hypertensive retinopathy, diabetes mellitus nor age related macular degeneration were included. Optical coherence tomography (OCT) was performed pre op, 7 days post op and 30-45 days post op phaco surgery.

Resultados / Results

After ANOVA analysis and macular and choroidal thickness measurements it was noted an increment in macular thickness in patients after 30 days but there was no correlation with the use o epinephrine. Choroidal thickness also did not show any increment in its thickness after phacoemulsification.

Discussão e Conclusões / Conclusion

In accordance to the literature, macular thickness increases after phaco surgery due to the influence of the inflammatory process induced by the procedure; Choroidal thickness, on the other hand, did not show any significant difference after 30 days of surgery. Some studies suggests that choroidal thickness may increase in earlier post op choroidal evaluation but none showed a significant increment in late evaluation. In this study, choroidal thickness maintained stable throughout the post op evaluation.
