BILATERAL CENTRAL SEROUS. PHOTOCOAGULATION TREATAMENT. CASE REPORT
Introdução / Purpose
Introduction: Central Serous Chorioretinopathy (CSC) is an important cause of central vision loss, more common in men aged 30-60 years and is associated with corticosteroid use, stress and genetic factors. An increase in choroidal vascular permeability is believed to occur, which would increase the hydrostatic pressure below the Retinal Pigment Epithelium (RPE) causing a localized and limited serous detachment of the retinal sensorineural epithelium and several therapeutic options have already been proposed, including laser photocoagulation.
Material e Método / Methods
43 years old male reporting metamorphopsia, photophobia for 3 months, which coincided with a period in which he assumed a leadership position at his job, and was under intense emotional stress. He had been diagnosed with Chronic Central Serous Chorioretinopathy (CCSC) since 2014. At the time, he presented AVCC of: OD: 20/40 and OS: 20/30. OCT of the macula from with a central thickness of OD: 480 and OS: 447. In the fluorescent retinography, he showed hyperfluorescence due to a defect in the perifoveal window in the OD, and hyperfluorescence by pooling in the inferior temporal perifoveal region in the OS.he underwent macular photocoagulation, with argon green laser, guided by fluorescein angiography, in both eyes. The laser used were Zeiss 532s diode laser with green light emission with wavelength of 532nm. The parameters used were application time of 200 ms, 50 μm size, range of 500 ms and power of 120 mW. The VOLK area centralis lens was used.
Resultados / Results
Total remission of bilateral serous edema with improvement of AV and metamorphopsia 4 months after treatment performed leaving discrete marks an without scotomas after remission
Discussão e Conclusões / Conclusion
The treatment of Central Serous Chorioretinopathy is still controversal but the serous exsudation can cause a permanent damage to the RPE and neurossensorial retina. Used the argon laser with complete remission of serous edema with low cost still has an important hole in the treatment of central serous
