CASE REPORT: POST-COVID 19 HYPERTENSIVE CHOROIDOPATHY IN A PUERPERAL PATIENT.

Introdução / Purpose

To illustrate an atypical case of bilateral serous retinal detachment in a patient diagnosed with post-Covid-19 pre-eclampsia.

Material e Método / Methods

Review of the patient's medical record.

Resultados / Results

A 25-year-old woman in her first pregnancy coming from Fortaleza-CE was admitted at 34 weeks and 1 day of gestation with reports of palpitation and dyspnea that started about 1 week before admission. These symptoms were associated with a baseline blood pressure of 180/120 mmhg with persistent peaks, headaches, and visual blurring. Patient presented a mild Covid-19 clinical picture with a positive RT-PCR test 07 weeks before admission. Cesarean delivery was performed 1 hour after admission. Patient continued with pressure changes after delivery. On the third postpartum day, she developed low bilateral visual acuity which was worse in the right eye. Visual deficiency persisted after sulfate therapy. A computed tomography scan revealed a normal cranium. Visual acuity with correction upon ophthalmologic examination: figures and shapes in the Right Eye (RE) and finger counts in front of the face in the Left Eye (LE). Biomicroscopy was normal in both eyes (BE). Intraocular pressure was 14/13 mmHg. Binocular Indirect Ophthalmoscopy: bilateral serous detachments with yellowish-white deposits at the level of the retinal pigment epithelium surrounding the macula with areas of the retinal crease in the LE. Portable retinography pictures showed exudative macular in BE with RD and macular optical coherence tomography (OCT) confirmed the presence of the detachment of the pigment epithelium in BE. On the 15th day of puerperium, serous detachments resolved spontaneously and bilaterally. This was confirmed by OCT. Patient progressed with improvement in visual acuity to 20/20 in BE.

There was visual improvement after clinical treatment of hypertension and resolution of pregnancy, even with c
horoidopathy and serous retinal detachment after pre-eclampsia associated with Covid-19.
