PNEUMATIC RETINOPEXY AND LASER PHOTOCOAGULATION FOR MACULAR RETINAL DETACHMENT SECONDARY TO OPTIC DISC COLOBOMA

A 41-year-old male patient was referred complaining of progressive worsening of visual acuity in the left eye in the past 10 months. He had a history of an untreated retinal detachment in his right eye, as well as use of an ocular hypotensive due to suspected glaucoma.
The BVCA was hand movement in the right eye and 20/30 in the left eye. Biomicroscopy and tonometry were performed, with results within normal limits. Fundus examination revealed that both optic discs were enlarged, with deep cupping occupying the entire disc, consistent with complete optic disc coloboma. There was no retinal and choroidal involvement. Ultrasonography showed partial agenesis of the anterior portion of both optic nerves, confirming the diagnosis of bilateral coloboma. The right eye showed total retinal detachment with pigmentary changes, compatible with a chronic condition. In the left eye, the macula presented an aspect suggestive of the presence of subretinal fluid, confirmed by optical coherence tomography.

The proposed treatment was the intravitreal injection of 0.3 ml of C3F8, associated with prophylactic laser photocoagulation of the retinal periphery and face-down head position. One week later, photocoagulation was performed around the optic disc. There was complete improvement of the serous macular detachment and there was no fluid recurrence. BVCA remained at 20/30.
