COATS – LIKE EXUDATIVE VITREORETINOPATHY IN RETINITIS PIGMENTOSA: A CASE REPORT

Introdução / Purpose

To report a case of coats like exudative vitreoretinopathy in a middle-aged brazilian woman with diagnosis of retinitis pigmentosa (RP).

Material e Método / Methods

Evaluation of ocular findings at RP diagnosis and at time of presentation, retinal imaging and treatment of coats-like exudative vitreoretinopathy.

Resultados / Results

A 50-year-old woman present to our outpatient clinic with a history of loss of vision in right eye (OD) of 1 year duration. About five years ago, she had undergone cataract surgery in this eye. Best correct visual acuity was light perception OD and 0.2 left eye (OS). The slit lamp biomicroscopy examination showed right pseudophakic eye and OS, cataract. Dilated fundoscopic exam revealed bone spikes OU and, in OD, retinal macular detachment extending to inferior temporal retina associated with lipid exudation. OS showed vitreous opacities, macular edema and vitreous hemorrhage in absorption. Wide field retinography, fundus autofluorescence and fluorescein angiography were performed and contributed to the diagnosis of Coats like retinitis pigmentosa. Intravitreal injection of anti-vascular endothelial growth factor was performed in OD and topical dorzolamide in OS. The patient is still under follow-up.

Discussão e Conclusões / Conclusion

Coats-like vitreoretinopathy is present in up to 5% of patients diagnosed with RP. Coats-like RP is distinct from Coats disease in that exudative pathology occurs exclusively in the setting of a coexisting RP diagnosis, is restricted to the infratemporal retina, can affect both eyes, and does not demonstrate a male sex bias. Early diagnosis and treatment may halt the progression of retinal diseases which possibly leads to a better prognosis. All patients diagnosed with RP should be evaluated with detailed fundus examination not to miss out any alterations in the periphery.
