MACULAR EDEMA AND ERM AFTER FACECTOMY IN A HIGH MYOPIC PACIENT: CASE REPORT.

Introdução / Purpose

High myopia is characterized by an axial length greater than 26mm. Surgical correction of high myopia can be done by corneal refractive surgery or by implantation of an intraocular lens (IOL). This IOL can be phakic or, depending on the patient's age and lens characteristics, a removal of the natural lens with an IOL implant in a capsular bag.

Material e Método / Methods

Female patient, 46 years old, white, assisted in July 2019 with high myopia (AL in RE of 30.70mm and in LE of 30.51mm), presented refraction of -16.75 SPH in the RE and -16.50 SPH in the LE and visual acuity (VA) 20/40 in both eyes (BE) with best correction. Facectomy and IOL implantation performed with a lens power of +1.00 in BE. She evolved 7 days after second eye surgery with uncorrected VA of 20/20 in BE and addition of +2.75 J1.

Resultados / Results

The patient remained asymptomatic for 24 months when she complained of low visual acuity (LVA) in the LE. On biomicroscopy, she presented Posterior Capsular Opacity and a capsulotomy was performed. After 12 days and without improvement in LVA, she returned for investigation, when retinography and OCT were performed, showing the appearance of epiretinal membrane and macular edema in the LE. She underwent pars plana vitrectomy with membranectomy plus intravitreal injection of anti-VEGF. She evolved postoperatively with visual and macular anatomy improvement on OCT. The patient is being followed up at this service to resolve the macular edema.

Discussão e Conclusões / Conclusion

Phacoemulsification allows good results in patients with high myopia and the complications observed are rare. In a systematic review of the literature, no similar cases to the one reported was found, which justifies the follow-up of more patients undergoing cataract surgery with IOL implantation to correct high myopia.
