EARLY REMOVAL OF METALLIC INTRAOCULAR FOREING BODIES: A CASE REPORT

Introdução / Purpose

The intrusion of intraocular foreign bodies usually occurs through direct penetrating trauma. Depending on the location and composition of the foreign body, the diagnosis can be made by direct observation or by imaging tests. Conventional radiographs, ultrasound, computed tomography, and magnetic resonance imaging are diagnostic tests. However, the nature of some foreign bodies may make it difficult to identify them in spite of the imaging tests used. The treatment of penetrating lesions varies according to the severity, extent and location of the lesion, in addition to the presence or absence of intraocular foreing body .The presence of these may result in orbital cellulitis, ocular lesions, endophthalmitis, adjacent skin lesions, partial loss of visual acuity or amaurosis, among other complications. The objective of this study was to report a clinical case of a 50 year old brown patient who arrived on duty with suspicion of a foreign body after a work accident in the right eye.

Material e Método / Methods

The patient had corneal, crystalline, posterior capsule, vitreitis, serous retinal detachment and significant loss of visual acuity. The ocular ultrasound did not find the intraocular foreing body, only evidenced the serous RD. On the other hand, the CT of orbits presented in right eye images with density compatible with metallic foreign body.

Resultados / Results

The patient underwent posterior vitrectomy via pars plana at the Hospital de Olhos de Feira de Santana-CLIHON / BA, where it was removed a hard-to-reach metallic intraocular foreing bodies with magnetic probe. The patient, already in the immediate postoperative period, evolved with remission of the infectious process and improvement of visual acuity.

Discussão e Conclusões / Conclusion

We opted for the precocity of the surgery in view of the risk of endophthalmitis evidenced by the current vitreitis and knowing that an early surgical approach is associated with a decrease in the risk of clinical infections in general.
