LIPEMIA RETINALIS: DESCRIPTIVE ANALYSIS AND OUTCOMES AFTER TREATMENT.
Introdução / Purpose
The objective of this study is to describe the retinal alterations in patients with the coexisting diagnosis of Lipemia Retinalis and hypertriglyceridemia. Also, identify the association between levels of systemic triglycerides and retinal clinical aspect.
Material e Método / Methods
Fundus images and fluorescein angiography were performed with wide-field retinography in addition to optical coherence tomography angiography (OCT-A) for a vascular morphological survey. The spectral-domain optical coherence tomography (SD-OCT) was used to identify changes in inner and outer retinal layers associated with Lipemia Retinalis.
Resultados / Results
After six weeks of treatment with Fenofibrate and a consequent triglycerides serum decrease from 4.000 mg/dL to 2.000 mg/dL, a new assessment was performed. OCT-A testing showed an enlargement of the foveal avascular zone (FAZ), probably because the patient had type 2 diabetes mellitus for three years. Fluorescein angiography was typically unremarkable. The wide-field fundus photography showed vessels with a creamy color appearance that normalized after metabolic control of triglycerides. SD-OCT testing revealed hyperreflective and engorged retinal vessels and white dots in inner nuclear and ganglion cell layer which may slowly dissipate throughout several months.
Discussão e Conclusões / Conclusion
Lipemia Retinalis is an ocular finding associated with high plasma triglycerides levels. Wide-field retinography and SD-OCT effectively detected retinal changes and suggested significant vascular and intra-retinal appearance improvements after treatment onset.
